29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a) Inline Room, 20 Francis Street, Keyport, NJ: An employee was exposed to amputation hazards. An employee was inspecting the quality of products produced on the C1 Thermal Former Machine when her left hand came in contact with the cutter. The employee sustained an amputation of the left hand pinky and partial amputation of the next three fingers. Observed on 10/01/2024  b) Tool Room, 20 Francis Street, Keyport, NJ: Employees were exposed to hazards associated with an unguarded drill press. Employees would operate the Bridgeport Drill press with the guard removed from the unit. Observed on or about 10/30/2024.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

An employee was inspecting the quality of products produced on the C1 Thermoformer unit when the left hand came in contact with the cutter. The employee sustained the a complete amputation of the pinky and partial amputation of the three other fingers. Observed on or about 10/01/2024  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
