This was demonstrated on or about October28, 2025, when employee(s) were manually cleaning up Cheney Lime and Cement Quicklime. Employee(s) were instructed to clean up materials that were leaking from a storage silo and had not been provided information regarding the potential hazards associated with the material. The employer failed to include as part of their hazard communication program information on the hazards of crystalline silica.

This was most recently demonstrated on or about October 28, 2024, when employee(s) were instructed to cleanup up Cheney Lime and Cement Quicklime, containing crystalline silica. The employer had not developed or implemented a written hazard communication program that included information being provided to employee(s) regarding the use of chemicals and substances that they could be exposed to as part of their job duties.

This was most recently demonstrated on or about October 28, 2024, and at times prior to when employees in the production area(s) were exposed to physical and health hazards; including but not limited to, toxic or hazardous substances and potentially hazardous atmospheres.
