29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about October 31, 2024, and at times prior thereto, in Hangar #2 warehouse, employees used a rolling ladder without it being inspected for possible defects prior to each shift.
