29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: a) On or about 10/16/2024, at the basement event center floor area of the site, Lockport, NY. Floor drain holes, that were 16x18 and 18x36 inches, were not covered to prevent trip and fall hazard. Drain holes were on basement floor, where there were active work by different trades. NO ABATEMENT CERTIFICATION REQUIRED
