A) 65 Tarrytown Road, White Plains - NY: An employee was working in a Versa lift and was not protected by any type of fall protective device or personal fall arrest system. Another employee was driving the vehicle while the exposed employee was in the lift, not tied off, and in an elevated position, on or about, October 31st, 2024.
