29 CFR 1910.23(b)(9):29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. a) On or about October 30, 2024, and at times prior; the employer exposed the employees and contractors to fall hazards, in that the only ladder access for the entire complex had a broken wall fixing bracket for a 18 foot fixed ladder that was used for roof access by employees and contractors.

29 CFR 1910.23(b)(10):29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced. a) On or about October 30, 2024: and at times prior, the employer exposed employees and contractors to fall hazards, in that the employer failed to lock the access door and tag the ladder "Dangerous: Do Not Use" resulting in contractors accessing the roof on a 18 foot fixed ladder with a broken wall fixing bracket.
