29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about October 30, 2024, the employer did not ensure that machine specific procedures for the control of hazardous energies on the lumber stacker were utilized prior to service and maintenance taking place. An employee had removed a fixed barrier guard in order to clear a jam on the equipment without utilizing the procedures. Hazardous energies present on the equipment include mechanical, electrical, and pneumatic.

29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about November 4, 2024, the employer did not ensure that a periodic inspection (annual) if the machine specific procedures for the control of hazardous energies had taken place. Employees are tasked with the service, maintenance, and operation of equipment including but not limited to lumber stackers, hoists, chain driven apparatus, saws, and mills. Hazardous energies present on equipment includes but is not limited to pneumatic, mechanical, hydraulic, and electrical.

29 CFR 1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: On or about November 4, 2024, the employer did not ensure that effective machine guarding was in place on the chain and sprocket of the lumber stacker. A motor driven chain and sprocket did not contain and enclosure to protect employees from the ingoing nip point where the chain meets the sprocket on the underside of the lumber stacker. Employees frequent the area to align lumber and store materials for production.
