29 CFR 1926.1203(d): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with �1926.1204 implemented at the construction site: Apartment Complex: On or about October 29, 2024, and at times prior thereto, employees entered crawl spaces through hatches on the sides of the apartment buildings without a developed and written permit confined space program.
