On or about October 26, 2024, in the facility, an employee was exposed to burns due to an unguarded hole approximately eighteen inches (18") by thirty-six (36") inches on the walking surface of the Mart Power Washer Hurricane 84, Serial number 6645. Employee(s) were exposed to a burn hazard when the protective covering over the hole had been removed from the walking surface which allowed an employee(s) foot to enter into the opening and reservoir that contained heated water and cleaning solution.
