29 CFR 1910.67(c)(2)(v): A body belt was not worn and lanyard was not attached to the boom or basket when working from an aerial lift: On or about November 04, 2024, at 344 E. Palm Drive, Homestead, Florida, an employee was exposed to ejection and fall hazards of approximately 12 feet in height while trimming a tree from an articulating aerial bucket without wearing a personal fall arrest or restraint system connected to the bucket's anchorage point.
