OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to fall hazards while utilizing a section of a portable metal extension ladder. On or about November 1, 2024, at a workplace located at N781 Curran Road, Denmark, Wisconsin in the Green House; employees were exposed to a fall hazard of approximately eight feet to the concrete floor below when installing shade clips around the perimeter of the bin area via a section of a portable metal extension ladder. The 75 degree angle of inclination for safe positioning of the ladder was not maintained to prevent accidental displacement.
