29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  Building Exterior:  An employee was exposed to a 6-foot fall while working on top a 40 yard garbage container. The employee was not using any type of fall protection, on or about 10/30/24

29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means shall be fully lowered, controls neutralized, and the brakes set to prevent movement.  a)  Outside of the warehouse: The load engaging means was not lowered, controls were not neutralized, and/or the brakes were not set to prevent movement when the operator of an industrial truck was dismounted and within 25 feet of the truck still in his view, on or about 10/30/24.
