29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) On or about October 31, 2024, employees working on and around the Qualheim Food Slicer (Model No. 400 & Serial No. 029321-D) were exposed to an amputation hazard, in-that the employer did not ensure the exit chute was properly guarded to prevent the operator from reaching into the point of operation.
