29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:   a. The Roof - On or about November 6, 2024, employees were framing the roof of an expansion of the existing structure. The employees did not have a safe means of access and egress as they had been using a forklift that was not always in the raised position and did not have fall protection to safely and reliably constitute safe access and egress or a means as prescribed by this standard. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a. The Roof - On or about November 6, 2024, employees were framing the roof of an expansion of the existing structure. The employees working at heights greater than 6 feet were not trained to recognize and mitigate fall hazards at the jobsite.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employees working at heights greater than 6 feet that had been trained on fall hazards were not documented through written certification.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
