Sound points of anchorage include structural members, but do not include standpipes, vents, other piping systems, or electrical conduit. Location: 1751 85th St. Brooklyn, NY. On or about 11/7/2024, a) Tie backs for the cornice hooks were not installed at right angles to the face of the building and there were not opposing angle tiebacks installed. The tiebacks for the cornice hooks were not secured to structurally sound point of anchorage on the building.

Employees stacked together three cement blocks on the sidewalk shed to access the extension ladder that was approximately 48 inches of break in elevation.
