29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   Worksite located at 7504 N. Oak Trafficway, Gladstone, Missouri- Employees were exposed to a fall hazard while working on a steep roof without the use of any  means of fall protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   Worksite located at 7504 N. Oak Trafficway, Gladstone, Missouri- Employees were exposed to a fall hazard while using a ladder that was raised at least 3 feet above the upper landing surface.
