Wayneport Road, Macedon, NY - Folding Area. On or about November 6, 2024, authorized employees did not utilize the documented procedures for the control of potentially hazardous energy. Employees were performing servicing activities such as  roller changes on equipment including but not limited to the 801s and Cheetah Envelope Presses. The machinery is equipped with multiple energy sources, including electric, pneumatic, and thermal. The employer did not to enforce specific LOTO procedures essential to prevent unexpected energizing, startup, or release of stored energy during servicing or maintenance of machinery and equipment.

The equipment, which is equipped with multiple sources of energy including electric, pneumatic and thermal was not rendered inoperable during servicing. The employer did not train its employees to ensure that the purpose and function of the LOTO/energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the LOTO were acquired by employees.

On or about November 6, 2024, workers operated the unguarded 801-10 Envelope press while exposed to point of operation, ingoing nip points, rotating parts and pinch points. The employer did not ensure workers were protected from contact with moving parts.   b) Information Packaging Corp., 1670 N. Wayneport Road, Macedon, NY - Folding Area. On or about November 6, 2024, workers operated the unguarded 801-11 Envelope Press while exposed to point of operation, ingoing nip points, rotating parts and pinch points. The employer did not ensure workers were protected from contact with moving parts.   c) Information Packaging Corp., 1670 N. Wayneport Road, Macedon, NY - Folding Area. On or about November 6, 2024, workers operated the unguarded 801-12 Envelope Press while exposed to point of operation, ingoing nip points, rotating parts and pinch points. The employer did not ensure workers were protected from contact with moving parts.   d) Information Packaging Corp., 1670 N. Wayneport Road, Macedon, NY - Folding Area. On or about October 23, 2024, workers were permitted to operate the Cheetah Envelope and Adhesive Press while exposed to rotating and moving parts such as rollers, conveyors, drives, feeding mechanisms, and pressure points. The employer did not ensure workers were protected from contact with moving parts.

Wayneport Road, Macedon, NY. Throughout the site, on or about November 6, 2024, workers utilize chemicals such as but no limited to parts cleaners, acetone, adhesives, defoamer, and battery acid that could result in injuries such as skin and/or eye irritation. Workers were not provided with training on the chemicals in their work area.
