29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) 1729 W. 166th Lane, Broomfield CO: On or about November 6, 2024, and at times prior, employees were exposed to struck by and crushing hazards when; prior to permitting an employee to operate a powered industrial truck, the employer did not ensure that each operator was competent to operate the JCB Forklift (Model 509.42F, PIN: JCB5CF7GVM3060491) safely.
