29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) West end of warehouse; On or about November 4, 2024, and dates prior, while using the AccurShear hydraulic shear (Model 837510, S/N 3614), operators and assistants were exposed to the shear blade and part hold downs in that the guard did not prevent access to the point of operation.
