29 CFR  1910.147(c)(4)(ii)(C):  The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them:  603 Hendee Street, Springfield, MA:  On and before November 7, 2024,  some energy control procedures (ECP) did not include specific procedural steps for the placing of lockout devices required for energy control.  The Kopex Oven ECP natural gas energy source stated to close valve, apply lockout device, and apply lock.  However,  the valve to close the gas flow had no lever and the type of device to use to lock out was not stipulated.   Also, the Extruder ECP air energy source for the take up section stated to shut valve and apply lock, but the valve was not lockable and a specific device was not stipulated.

29 CFR  1910.147(c)(4)(ii)(D):  The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:  603 Hendee Street, Springfield, MA:  On and before November 7, 2024, the verification steps in the energy control procedures (ECP) were not sufficient as they only included a generic statement to start the machine.  The Extruder ECP air energy source for the take up section stated to bleed off residual air pressure, but there were no details as how to do that or to verify the isolation/bleed off.    Also, there were no specific steps on how to verify the isolation of the natural gas energy source for the Kopex Oven ECP.

29 CFR  1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  603 Hendee Street, Springfield, MA:  On and for more than two years before November 7, 2024, the employer had not performed a comprehensive periodic inspection of energy control procedures for all multi-energy sources machines, including the Kopex Oven machine, at least annually, to verify that each procedure was adequate and being properly used by employees.  *Abatement Notes:  A periodic inspection shall include the following elements:  - Appointment of an authorized employee to inspect each local procedure for inaccuracies during an energy control task - A review by inspector with each authorized employee, of that employee's responsibilities under each energy control procedure they are authorized to use - Certification that the periodic inspections have been performed by identifying the machines or equipment on which the energy control procedures were being utilized, the dates of the inspections, the employees included in the inspections, and the persons performing the inspections *Abatement notes are intended only to provide clarity regarding existing requirements under the law or agency policies. Abatement notes do not create (or diminish) legal obligations under the Occupational Safety and Health Act.
