29 CFR 1926.501(b)(7)(i): Each employee at the edge of an excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, or barricades when the excavations were not readily seen because of plant growth or other visual barrier: (a) On or about November 7, 2024, at the jobsite, employees walking near the edge of the bell hole to access porta potties were not protected from falling from heights of more than 6 feet in that part of the safety barricade for the bell hole area had been taken down.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) On or about November 7, 2024, at the jobsite, one employee was observed working on a water pump while standing on a shelf that had been excavated into the sloped wall of the trench and was not protected from cave-ins in that the trench had not been sloped as needed for class C soil.
