Location: Workplace located at 256-271 Marginal Street East Boston On January 7, 2024, employees were exposed to hazards associated with rigging failure such as but not limited being caught-between the ground and a falling 220,000 lbs. load and being struck-by falling components, while working with nylon slings that were inadequately protected from the edge of the steel dock that was being lifted. Or in the alternative. 29 CFR 1926.251(c)(9) Slings were not padded or protected from the sharp edges of their loads: On January 7, 2024, employees were exposed to hazards associated with rigging failure such as but not limited being caught-between the ground and a falling 220,000 lbs. load and being struck-by falling components, while working with nylon slings that were inadequately protected from the edge of the steel dock that was being lifted. This citation is based on the information available. The employer also possesses additional information that OSHA has requested via a subpoena served December 3, 2024, that the employer did not provide, such as but not limited to; items (1). All witness statements and notes of witness statements concerning the incident on November 7, 2024, the worksite located at 256-271 Marginal Street East Boston. (2) All photographs, videos audio recordings or transcriptions, or any other recording relating to the jobsite. (3) All daily reports concerning the jobsite. (4) All schedules of work concerning the worksite. (9) All communications (including emails, texts, electronic correspondence or otherwise) from November 6, 2024, to present concerning the Job Site. (12) All documents concerning safety inspections conducted at or for the Job Site.
