29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a) On or about November 4, 2024, an employee installing a surveillance camera on a traffic signal mast from a LEGUAN 225 aerial lift was not protected from the fall hazard of approximately 14-feet; the employer did not ensure that the employee was protected while working from an aerial lift by use of a personal fall arrest system to include but not limited to, a harness and lanyard attached to the basket.

29 CFR 1926.503(c): "Retraining." When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. Circumstances where retraining is required include, but are not limited to, situations where: 29 CFR 1926.503(c)(2): Changes in the types of fall protection systems or equipment to be used render previous training obsolete. a) On or about November 4, 2024, an employee installing a surveillance camera on a traffic signal mast from a LEGUAN 225 aerial lift was not protected from the fall hazard of approximately 14-feet; the employer did not ensure that the employee was retrained after the purchase of the LEGUAN 225 aerial lift to ensure the affected employees understanding of when to wear fall protection.
