29 CFR 1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it: a. Site- Employees were exposed to hazards associated with structural failure when a suspended scaffold system that they were working from contained ladders and ladder jack brackets with rated capacities which were exceeded by the weight of scaffold components, personnel, and equipment. Additional structural concerns included deviations from proposed erection plans, including but not limited to, different sized ladders, use of different sized ladder jack brackets, different sized planks, planks being unsecured, lack of cleats on planks, lack of anti-slip bars on each end of platform, lack of spreader bar between tie points, use of type 1 ladders instead of type 1A, use of wing nuts instead of lock nuts, guardrails and wind screen not factored into engineered version, less vertical members used than plans called for, exceeding height restrictions, planks overhanging supports excessively, and the system itself not being anchored to the roof. Ladder-jacks were not used properly for their intended use by the manufacturer. Ladders were not used in the intended use per the manufacturer.
