29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  Location - Jobsite;  On or about 10/11/2024, employees were not instructed in recognizing hazards or in complying with the equipment manufacturer's operating instructions in the following instances:  Instance a: Employees climbed on the lift basket to access the roof, contrary to the manufacturer's instruction to "keep both feet firmly positioned on the platform floor at all times."  Instance b: The lift was elevated while the rear tires were positioned on loose building materials to level it against a curb, violating the instruction to "ensure machine is positioned on a firm uniform surface before elevating platform."
