29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 6, 2024, employees engaged in commercial roofing activities, located at 3339 109th Plaza, Omaha, NE 68164, were exposed to fall hazards in that the employees were working in excess of 6-feet while conducting the roofing operations without fall protection.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about November 6, 2024, employees engaged in commercial roofing activities, located at 3339 109th Plaza, Omaha, NE 68164, were exposed to fall hazards in that the employees were accessing and egressing a roof using a ladder that did not extend 3-feet above the surface that was being accessed.
