29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: (a) Building 135, D Bay - On or about November 07, 2024, for employee setting up a crane to move 5,500 pounds breech block cannon by lowering steel ropes from an overhead crane using a pendant remote while standing directly below the ropes. The employee did not have head protection.

29 CFR 1910.179(j)(2)(i): All functional operating mechanisms for maladjustment interfering with proper operation were not inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections: (a) Building 135, D Bay - On or about November 07, 2025 employees were required to operate a P&H overhead crane to lift a 5,500 pound M284/M776 breech block cannon. The employer did not ensure that daily inspections of the P&H overhead crane were conducted prior to use.

The cannon was not secured and properly balance when lifted, causing the cannon to pivot and roll onto the employee.
