29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 11/05/2024, at 15680 SW 127th Avenue, Miami, Florida, an employee was exposed to a 25-foot fall and ejection hazards when engaged in the installation of electrical lights to a concrete pole while using an aerial basket without a harness or belt with lanyard connected to the boom or basket of the equipment.
