29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface had side rails that extend at least 3 feet (0.9 m) above the upper landing surface.   a) On or about November 7, 2024, at the southwest corner of the exterior patio area: employees were exposed to a 12'-6" fall hazard while using a Werner extension ladder to access the roof to repair a leak, in that the side rails extended approximately 20 inches passed the roof landing.

29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) On or about November 7, 2024, at the East side of the back middle area of the warehouse roof: employees were exposed to a 18'-6" fall hazard while fixing a roof leak and squeegeeing water off the edge of a high flat roof, in that, fall protection was not used.  b) On or about November 7, 2024, in the northeast corner of the warehouse: employees were exposed to a 9'-8" fall hazard while employees operated a vertical upstroke baler Economy Baling Press (Style#172 Serial# 56461) to create bales of clothing by loading clothing into the pit 9'-8" deep 6' long and 2'7" wide, in that, fall protection was not used.  The Salvation Army of Mercer County was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.28(b)(1)(i), which was contained in OSHA inspection number 1460103, citation number 1, item number 1a and was affirmed as a final order on 06-04-2020, with respect to a workplace located at 818 Mercer Street Princeton, WV 24740.
