29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Under-construction single-family home; On or about November 6, 2024, the side rails of a Cosco model 20918T1AS multi-position aluminum ladder, configured as an extension ladder and used by employees to gain access to the rooftop, did not extend at least 3-feet above the landing surface, was not secured to prevent displacement, and grab handles were not provided.
