29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: On or about November 04, 2024, an employee working from a scaffold was exposed to a fall hazard of 17.6 feet above concrete while completing formwork installation at a commercial jobsite located at 2049 Martin Luther King Jr Dr, CLEVELAND, OH 44106. The employer did not ensure the employee's personal fall arrest system anchor was capable of supporting a load of 5,000 pounds and installed per manufacturer's instructions which led to the fall and inpatient hospitalization of an employee.

29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards: On or about November 04, 2024, the employer did not ensure that employees completing formwork installation at a commercial jobsite located at 2049 Martin Luther King Jr Dr, CLEVELAND, OH 44106, were tied-off to an anchor point capable of supporting at least twice the potential impact load of an employee's fall or a designated anchorage point. Employees were exposed to falls greater than 6 feet, in that an employee was tied off from a positioning device directly to a formwork panel and the employee fell 17.6 feet to concrete when the unrated panel failed as an anchor point leading to an inpatient hospitalization.
