29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. Location - Woodworking / production room. - Dustek dust collector and electrical box near upcut saws. On or about 11/12/2024, combustible wood dust was allowed to accumulate on the floor around and behind the enclosureless Dustek dust collectors, including in an electrical control box for the dust collector.

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: Location - Woodworking / production room - Near upcut saw and in sanding area. On or about 11/12/2024, the SawStop Cabinet Saws were not guarded by a hood to protect employees from the sharp blade, exposing employees to laceration and amputation injuries.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. Location - Woodworking / production room (Electrical box between Dustek dust collector and upcut saws) On or about 11/12/2024, the Westinghouse electric box that controls the Dustek dust collector had an open knockout hole in the top of the electrical box covering, allowing for wood dust particles to enter the box, exposing employees to potential combustible dust explosion and fire hazards.
