Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  On or about November 12, 2024, the employer did not ensure that material was stacked in a secure manner to prevent sliding or collapse.  Employees were exposed to crushing hazards from a pallet of material that had slid and was unstable.
