29 CFR 1910.176(b):Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. On or about November 12, 2024, the employer did not ensure that material was stacked in a secure manner to prevent sliding or collapse. Employees were exposed to crushing hazards from a pallet of material that had slid and was unstable.

29 CFR 1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. On or about November 4, 2024, the employer did not certify that each operator had been trained and evaluated as required. The employer did not have certification for an employee who operated the stand-up forklift(s). Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
