29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): (a) Multi-story residential structure, southwest exterior; On or about November 7, 2024, an employee installing chimney venting from a 4-tiered fabricated frame scaffold, placed on the lower rooftop of the home, was working from the top platform, approximately 21-feet above a lower level, which was not completely decked between the front uprights and guardrail supports.

(b) Multi-story residential structure, southwest exterior; On or about November 7, 2024, employees were not protected from falling approximately 10-feet to a lower level while working from the upper rooftop to install chimney venting. (c) Multi-story residential structure, southwest exterior; On or about November 7, 2024, an employee was not protected from falling approximately 22.5-feet to a lower level while working from the top of a brick chimney to install chimney venting.
