29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a residential worksite located at 1001 NE 73rd Pl, Gladstone MO -- employee working on the ground directly underneath where old shingles and debris were being tossed down was not wearing any head protection. This exposed the employee to head injuries. This was documented on or about 11/12/2024.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 1001 NE 73rd Pl, Gladstone MO -- employees engaged in roofing duties using pitchforks, hammers and other small hand tools were not wearing eye protection. The employees were in the process of tearing off the existing roof. This exposed employees to getting shrapnel and/or debris in their eyes causing injury. This was documented on or about 11/12/2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a residential worksite located at 1001 NE 73rd Pl, Gladstone MO -- employees engaged in roofing duties were not wearing fall protection. The employer had no fall protections system in place. This exposed the employees to fall hazards ranging from 8.6' to 15.9; ground to roof's edge/eave. This was documented on or about 11/12/2024.
