OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards due to employees operating the control switch while folding the platform and flip-over. Location: 780 Ocean Street, Hyannis MA 02601 liftgate area of truck. On or about November 6, 2024, employees were exposed to amputation hazards while operating the control switch while folding the Maxon Liftgate. One employee was folding the platform and flip-over while the other employee was operating the control switch resulting in a partial finger amputation.
