29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard: 5.1.6: Eyewashes shall be capable of delivering flushing fluid to the eyes not less than 1.5 liters per minute (0.4 gpm) for 15 minutes. If shut off valves are installed in the supply line for maintenance purposes, provisions shall be made to prevent unauthorized shut off.  Champion Kitchen: On or about November 13, 2024, and at times prior thereto, the employer failed to provide an emergency eye wash station, providing 15 minutes of flush time, where employees used corrosive chemicals, which potentially exposed employees to corneal or retinal burns.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  Facilities: On or about November 13, 2024, and at times prior thereto, the employer failed to conduct a workplace hazard assessment and create a written certificate for employees who conducted equipment preventive maintenance operations which required safety glasses/goggles and chemical resistant gloves, which potentially exposed employees to contusions, lacerations, corneal scratches and dermal irritation.

29 CFR  1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  Facilities: On or about November 13, 2024, and at times prior thereto, the employees conducted wall sanding operations and tile removal/installation operations under a voluntary use respirator program.  The employer had not provided the respirator user(s) with the information contained in Appendix D which potentially exposed employees to respirable silica inhalation hazards.

29 CFR  1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were prominently displayed on the container or readily available in the work area throughout each work shift:   Facilities:  On or about November 13, 2024, and at times prior thereto, the employees used hazardous chemicals in separate secondary spray containers without manufacturer warning properties listed and without having legible chemical names. The employer had not ensured chemical names and proper warning labels were attached to the secondary containers which potentially exposed employees to retinal or dermal irritations.
