29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Warehouse - On or about November 21, 2024, the employer did not ensure effective guarding was provided on the scrap metal conveyor belt. An employee required to load scrap metal onto the conveyor belt while standing in front of an unguarded conveyor belt roller was exposed to hazards associated with ingoing nip points and rotating parts. b.) 29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: A.) Warehouse - On or about November 21, 2024, the employer failed to ensure a chain and sprocket assembly on the scrap metal processing conveyor system were enclosed with a guard. An employee was required to stand near a chain and sprocket assembly that was not eclosed with a guard to scrape ferrous metal into a plastic container, which exposed the employee to pinch point, crush, and amputations hazards.
