29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about November 14, 2024, at the jobsite: the employer's ladder was only extended approximately 2 feet past the roof landing, while being used  to gain access to a roof approximately 14'5" high, with a pitch of 5/12, exposing employees to fall hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about November 14, 2024, at the jobsite: the employer did not enforce the use of fall protection while employees installed shingles on a 10'2" high roof, with a pitch of 5/12, exposing employees to fall hazards.  The Orchids Builders, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1659563, citation number 1, item number 1 and was affirmed as a final order on 9/7/2023, with respect to a workplace located at 2511 Windermere Reserve Ct. Windermere, FL 34786.
