29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about November 14, 2024, it was observed that employees performing roofing activities in residential construction located at 5907 Pacific Street, Omaha, Nebraska were exposed to fall hazards in that the employer did not ensure employees working on a steep roof approximately 15 feet, 9 inches above the next lower level were trained to identified fall hazards and appropriate wear of personal fall protection.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: On or about November 14, 2024, employees performing roofing activities at a residential construction located at 5907 Pacific Street, Omaha, Nebraska were exposed to fall hazards in that employees accessed the roof using a multipurpose aluminum ladder that was incorrectly angled greater than the 4:1 ratio as required by the standard. The ladder angle horizontal distance measured approximately 10.93 feet as measured from directly below the access point on the roof to the base of the ladder, which was approximately 2:1.
