29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 14301 E Arapahoe Rd, Centennial, CO 80112: On or about 11/14/2024, the employer did not ensure that employees engaged in reroofing on a steep roof with unprotected edges were protected from falling by a fall protection system. This condition exposed employees to falls in excess of 25 feet.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: (a)14301 E Arapahoe Rd, Centennial, CO 80112: On or about 11/14/2024, employees exposed to falls in excess of 25 feet while engaged in reroofing were not trained on procedures to be followed in order to minimize these hazards. Employees were observed with no fall protection and incorrect use of fall protection equipment.
