On or about November 14, 2024, at the facility located at 211 South Lawson Drive, Green Lake, Wisconsin; an employee was operating a Genie Aerial Lift (Model No.: S65Trax, Serial No.: S60TX16H35963) without wearing fall protection while elevated to a height of approximately 20 feet, exposing them to potential fall hazards.
