29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Worksite, Screening Plant, Titan1800 Powerscreen Hopper: On or about November 7, 2024, an employee was assisting the foreman with removing the bolts on the collapsible rear wall of the hopper when the hopper fell and stuck the employee. The employee was not trained in the avoidance of unsafe conditions.
