29 CFR 1926.451(g)(4)(ii): The top edge height on all suspended scaffolds where both a guardrail and a personal fall arrest system were not between 36 inches (0.9 m) and 45 inches (1.2 m) above the platform surface :Note: For supported scaffolds manufactured and placed in service before January 1, 2000, and on all suspended scaffolds where both a guardrail and a personal fall arrest system are required, the top edge height should be between 36 inches (0.9 m) and 45 a.) 5436 Walnut St, Pittsburgh, PA 15232, On or about November 11, 2024, the employer failed to ensure the supported scaffold was free and clear of potential fall hazards during plasma steel cutting activities. The tubular welded frame scaffold working platform had an exposed unguarded edge exposing employees to fall hazards of up to 12 feet to the lower floor level.
