29 CFR 1910.178(n)(6):The driver shall be required to look in the direction of, and keep a clear view of the path of travel. a) On or about November 11, 2024, the employer did not ensure that an employee operating a Crown Walkie (Model: PE 4500-80 SN: 10075650) was looking in the direction of travel resulting in the employee striking the guardrail and being thrown from the walkie.

29 CFR 1910.178(q)(7): Maintenance of industrial trucks. Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. a) On or about November 11, 2024, the employer did not ensure powered industrial equipment including but not limited to the Crown Walkie (Model: PE 4500-80 SN: 10075650) was being inspected prior to being operated.
