29 CFR  1926.451(a)(1): Scaffold(s) and scaffold(s) component(s) were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it:  a) New Residential Home: On or about November 8, 2024, the employer exposed employees to fall hazards of 20 feet or greater to the ground below, in that the Werner Fiberglass Multi Section Extension ladder component  of the ladder jack scaffold, with a maximum load capacity of 375 pounds  fractured, and destabilized while employees began roof shingle preparation, and installation activities from the elevated walking platform.
