On or about October 13, 2023 and times prior to, Lindsey Construction was installing roof trusses at a jobsite located at 905 West Walnut Street in Ellington, Missouri. The employer did not ensure operators of a Class VII variable reach rough terrain forklift, Lull, model 644D, were trained in the safe operations of the powered industrial truck. Employees were exposed to fall hazards while working inside Haugen attachment model MWP4x8, serial # M0014535.
