29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location: Joanna Road, Avon, MA-Jobsite Instance #1: On November 14, 2024 a worker was working approximately 25 feet from the ground without any fall protection system in place fell to the ground. Instance #2: On November 15, 2024, workers were exposed to fall hazards of up to 25 feet to the ground below while conducting roof truss activities where fall protection was not used.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. LOCATION: Joanna Road, Avon, MA-Jobsite Instance #1: On November 14, 2024, an employee fell approximately 25 feet from the ground while conducting truss installation, where fall protection training was not provided to the employee to recognize and minimize fall hazards. Instance #2: On November 15, 2024 employees working approximately 25 feet from the ground while conducting truss installation, where fall protection training was not provided to the employees to recognize and minimize fall hazards.
