Employees engaged in the refurbishment of the middle metal clad switch gear cabinet were exposed to falls of approximately eight feet in that the fall arrest systems worn by employees were rigged in a manner that would not prevent a free fall of more than six feet, nor contact any lower level.

Employees engaged in the refurbishment of the middle metal clad switch gear cabinet were exposed to electrical shock hazards associated with contact with 12kV in that the BK-2M Bus was not deenergized and grounded or effectively guarded.

Employees engaged in the refurbishment of the middle metal clad switch gear cabinet were exposed to electrical shock hazards associated with contact with 12kV in that the electrical energy feeding the BK-2M Bus was not verified to be isolated.
