29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve.   (a) Facility at 15 Branch Pike, SMITHFIELD, RI: On or about January 27, 2025, a half-face elastomeric air-purifying respirator with dual P100 cartridges was hanging from its straps on a metal hook above a dusty, cluttered workbench. The respirator was not stored in a clean, sealed container or bag, and was positioned in an area where it was exposed to dust, tool residue, and potential chemical contaminants.

(a) Facility at 15 Branch Pike, SMITHFIELD, RI: On or about February 7, 2025, the employer did not establish and implement a written respiratory protection program where employees voluntarily wore elastomeric respirators while tasked with cutting, grinding or otherwise modifying silica containing material(s), including but not limited to granite.

(a) Facility at 15 Branch Pike, SMITHFIELD, RI: On or about February 7, 2025, the employer did not establish a written exposure control plan describing procedures, engineering controls, work practices, or protective equipment used to limit employee exposure to respirable crystalline silica.  Structural Stone LLC was previously cited for a violation of this occupational safety and health standard, 1910.1053(f)(2)(i), which was contained in OSHA inspection number 1726361 , citation number 1, item number 4b and was affirmed as a final order on 07/24/2024, with respect to a workplace located at 285 Smith Street in NORTH KINGSTOWN, RI 02852.
